Night leg pain has three main vascular causes: venous insufficiency and varicose veins (heaviness and cramps that ease when you raise the legs), advanced arterial disease (foot pain that eases when you hang the leg over the bed), and deep vein thrombosis (pain with sudden swelling). Foot pain that wakes you and eases when the leg hangs down is a warning sign that needs prompt arterial assessment.
Many people wake up with leg pain or cramps. Most cases are minor, but the type of pain and what relieves it can reveal an important vascular cause.
1. Night cramps
A sudden painful tightening of the calf muscle lasting minutes, common with fatigue, dehydration, low minerals or some medicines. It is usually not vascular, though it is more common in people with varicose veins.
2. Venous insufficiency and varicose veins
Heaviness, aching, burning and tingling in the legs, worse after a long day standing, and better when you raise your legs. Itching and ankle swelling may accompany it. This type improves a lot with varicose vein treatment.
3. Advanced arterial disease (rest pain)
This is the one not to ignore. When leg arteries are severely narrowed, the foot doesn't get enough blood even at rest. Patients feel pain in the foot or toes at night that improves when they hang the leg over the side of the bed or sleep sitting up, because gravity helps blood reach the foot.
This stage is often preceded by calf pain on walking that eases with rest, called intermittent claudication. See peripheral artery disease.
4. Deep vein thrombosis
Calf pain and tightness with swelling and warmth, usually in one leg, which can start at any time. It needs an urgent Doppler.
Non-vascular causes
Such as restless legs syndrome, diabetic nerve damage (burning and tingling), and spinal problems radiating to the leg. That is why an examination is needed to separate the causes.
How is it diagnosed?
- Checking foot pulses and the ankle-brachial index to assess the arteries.
- Arterial or venous Doppler depending on symptoms.
- Blood sugar and mineral tests when needed.
What to do now
Note when the pain starts, exactly where, and what relieves it: raising the leg, lowering it, or moving. This helps the doctor a lot. If the pain is in the foot and eases when the leg hangs down, don't delay.
Frequently asked questions
Why does foot pain improve when I hang my leg over the bed?
Gravity helps blood reach the foot when the arteries are severely narrowed. It is a sign of poor blood supply that needs urgent assessment.
Are night cramps dangerous?
Usually not, but if they are frequent or come with swelling, varicose veins or pain on walking, get checked.
Can varicose veins cause pain at night?
Yes, they can cause heaviness, cramps and burning that improve when you raise your legs.
